There are scenes of strong gory violence as people are mutilated and their bodies torn apart. In a sustained scene of sadistic violence, a young woman is repeatedly stabbed and then scalped with a pair of scissors. Her body is then pulled apart, with salt and bleach rubbed into her wounds, resulting in strong gory injury detail. In a similarly visceral sequence, the serial killer pours acid into a young woman's face, which melts, before she is repeatedly bludgeoned, her chest opened up and her heart extracted. The clown slashes at a young man's penis with a knife before tearing it from the victim's body and smearing it across a car window for the man's girlfriend to see. Other violence includes bloody shootings, beatings, stabbings, throat slashings and eye gouging.

There are scenes of strong sustained threat throughout as a serial killer stalks and menaces his victims. There are scenes of drug misuse, including a young couple snorting cocaine, as well as a young woman appearing inebriated following her drink being laced with MDMA after she has already mixed alcohol and Xanax. There are also verbal references to suicide and a father's 'abusive' behaviour towards his daughter.
